IS IT ECZEMA? MAYBE NOT!

In pediatric dermatology clinics we see kids that have been treated way too long for “eczema” as their more serious skin disease was misdiagnosed.

Morphea (Localized Scleroderma) is an autoimmune skin disease characterized by an area of inflammation, thickening and hardening of the skin due to increased collagen deposition. Some forms of Morphea can affect the underlying areas including the fat, muscle, fascia and bone sometimes leading to underdevelopment of bone or impaired joint movement (swipe left).

Morphea incidence is about 1 per 100,000 children, 3x more common in females. The cause seem to be genetic factors + external factors such as trauma or infection.

Diagnosis is made by a pediatric dermatologist, but a biopsy can help confirm the diagnosis. A number of autoimmune labs may also be ordered.

Treatment involves topical therapy, phototherapy, or systemic therapy including immunosuppressive medication (Methotrexate, systemic corticosteroids, and others) depending on type and severity of the disease. Some children with severe or widespread disease need to be on heavy immunosuppressive medication under the care of a pediatric dermatologist and a pediatric rheumatologist.

Milder forms of Morphea tend to become inactive within 3–5 years but childhood Morphea can relapse. Children are usually kept on extended treatment to decrease risk of relapse.
